Key Responsibilities- Provide comprehensive primary care to patients, including health promotion, disease prevention, and management of acute and chronic conditions.
- Work autonomously to diagnose, order, and interpret diagnostic tests, prescribe medications, and perform procedures within the scope of practice.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans that reflect the patient's goals and preferences.
- Provide education and support to patients and their families to promote health literacy and self-management.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Develop and maintain strong relationships with community healthcare providers to ensure seamless transitions of care.
- Current registration with the College of Nurses of Ontario as an NP (adult) or NP (primary health care).
- Master's degree in Nursing/Nurse Practitioner (MN-NP) or Master's of Science in Nursing with successful completion of a post-Master's Nurse Practitioner Diploma from a recognized school of graduate studies.
- Minimum 2 years of relevant experience in home/community care, acute care, chronic disease management, health promotion, palliative care, or mental health.
- Excellent clinical leadership and communication skills, with the ability to work independently and collaboratively as part of a team.
- Strong critical thinking and problem-solving skills, with the ability to navigate complex situations and prioritize tasks effectively.
