Nurse Practitioner

We are seeking a highly skilled and experienced Nurse Practitioner to join our team at peopleCare Communities. As a key member of our healthcare team, you will play a vital role in delivering high-quality care to our residents.
Key Responsibilities- Collaborate with the Director of Care to develop and implement resident care plans.
- Assess, diagnose, and evaluate resident conditions, and provide treatment and follow-up care as needed.
- Provide clinical leadership and education to nursing and personal support care team members.
- Monitor resident health and note any changes in conditions.
- Maintain accurate and up-to-date resident records.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Nurse Practitioner Certification from a recognized program.
- Current registration with the College of Nurses of Ontario as Registered Nurse in the Extended Class or RN (EC), in good standing.
- Advanced knowledge and decision-making skills in assessment, diagnosis, and healthcare management.
- Experience in Long-Term Care and working with seniors and unregulated care providers.
- Excellent leadership, communication, and interpersonal skills.
