About the Opportunity

We are seeking a highly skilled and compassionate Patient Access Navigator to join our team at Nova Scotia Health Authority. In this role, you will play a critical part in facilitating access to Mental Health and Addictions services across the province.

As a Patient Access Navigator, you will be responsible for utilizing appropriate screening tools, risk assessment, clinical triage and disposition, brief supportive counselling and psychoeducation, consultation, and information sharing to support individuals in need. You will also build working relationships, assist with problem solving, and support individuals while they learn to self-navigate the healthcare system.

The ideal candidate will possess a Bachelor's degree in Social Work, registration with the Nova Scotia College of Social Workers, and three years of related experience in Mental Health and Addictions in a healthcare setting. You will also have experience working with individuals with Mental Health and/or substance use/gambling concerns, high degree of motivational interviewing skills and engagement techniques, and knowledge of harm reduction strategies and stages of change theory.

About You
  • Experience working with individuals with Mental Health and/or substance use/gambling concerns
  • High degree of motivational interviewing skills and engagement techniques
  • Knowledge of harm reduction strategies and stages of change theory
  • Demonstrated awareness of community resources and programs and community development methods
  • Demonstrated knowledge of recovery-oriented practice & patient and family-centered care
  • Non-Violent Crisis Intervention, Mental Health First Aid an asset, or committing to complete these courses
  • Working knowledge of DSM
  • Ability to navigate and de-escalate situations and offer crisis support using appropriate methods when needed
  • Demonstrated knowledge of cultural sensitivity and cultural competency
  • Competencies in other languages an asset, French preferred
Hours and Compensation

This is a Permanent Full-Time position, 75 Hours Bi-Weekly, with a competitive hourly rate of $42.89. Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
