Job Summary

We are seeking a highly skilled Registered Nurse/Registered Psychiatric Nurse to join our team at Fraser Health. As a Community Care Specialist, you will work collaboratively with an interdisciplinary team to provide comprehensive care to clients with complex or chronic illnesses, chronic pain, mental health, and/or concurrent disorders.

Key Responsibilities
  • Conduct initial and ongoing client assessments to identify client needs and develop individualized care plans
  • Provide nursing care through therapeutic interventions, including counseling and group facilitation
  • Collaborate with clients, families, and healthcare providers to ensure seamless transitions across the healthcare continuum
  • Develop and implement educational programs to promote client health and well-being
  • Maintain accurate and comprehensive clinical records
Requirements
  • Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives
  • One year recent related experience providing treatment services to individuals with complex or chronic illness, chronic pain, mental health, and/or concurrent disorders
  • Valid BC Driver's license and access to a personal vehicle for business-related purposes
