In accordance with the Mission, Vision and Values of Provincial Health Services Authority, patient safety is a top priority and a responsibility shared by everyone at PHSA. As a result, the requirement to continuously improve quality and safety is inherent in all aspects of this position.
Key Responsibilities- Establish Therapeutic Relationships: Develop and maintain effective relationships with patients and their families to provide emotional support and assess their needs.
- Assess and Identify Patient Needs: Conduct thorough assessments to identify patients' actual and potential strengths and limitations, including physiological, psychological, social-cultural, and spiritual needs.
- Develop and Implement Care Plans: Contribute to the development and modification of individualized care plans, identifying patient priorities and suggesting revisions as needed.
- Perform Nursing Procedures: Carry out various nursing procedures, such as taking blood pressures, heights, and weights, and communicating patient information to the healthcare team.
- Monitor Patient Status: Continuously monitor patients' status in relation to anticipated outcomes and evaluate the effectiveness of nursing interventions.
- Maintain Accurate Records: Maintain complete and accurate computerized and paper records, tracking results, consults, and documentation in accordance with established procedures and policies.
- Medication Administration: Review medication administration records, check with the nurse-in-charge or physician regarding concerns, and store patient medications according to standards and agency directives.
- Deliver Medications: Deliver medications consistent with the written care plan, physician orders, and established procedures, assessing their effectiveness and documenting all patient medications.
- Assist with Examinations and Treatments: Assist RNs/RPNs and physicians in performing various examinations, treatments, and diagnostic procedures.
- Collaborate with Treatment Team: Work with the treatment team to assist patients with their program requirements, monitoring program requirements and using motivational interviewing techniques.
- Ensure Infection Control: Ensure that infection control protocols are followed by all patients and personnel, handling, storing, and disposing of workplace hazardous materials and sharps in accordance with established procedures.
- Perform Security Functions: Perform regular security functions to maintain the safety and security of patients, staff, families, and the general public, taking immediate action to mitigate risks and reporting all real and potential risk issues.
- Qualifications: Graduation from a recognized program for Practical Nurses, plus 1 year of experience working with offenders and/or in secure healthcare settings, or an equivalent combination of education, training, and experience. Current full practicing licensure with the British Columbia College of Nursing Professionals (BCCNP).
