Licensed Practical Nurse

In accordance with the Mission, Vision and Values of Provincial Health Services Authority, patient safety is a top priority. As a Licensed Practical Nurse, you will work collaboratively with the interprofessional team to provide healthcare services to patients with physical and/or mental health, substance dependence, concurrent disorder, and chronic disease.
Key Responsibilities- Establish therapeutic relationships with patients and families, assessing and identifying their actual and potential strengths and limitations, including physiological, psychological, social-cultural, and spiritual needs.
- Contribute to the development and modification of individualized care plans, identifying patient priorities and suggesting revisions as required.
- Perform various nursing procedures, such as taking blood pressures, heights, and weights, and communicating patient information to in-charge nurses, physicians, and other team members.
- Monitor patient status and evaluate the effectiveness of nursing interventions, reporting unanticipated changes and concerns to the nurse-in-charge.
- Maintain accurate and complete computerized and paper records, tracking results, consults, documentation, and inputting data and documentation in accordance with established procedures and policies.
- Review medication administration records and check with the nurse-in-charge or physician regarding any concerns.
- Store patient medications according to standards and agency directives.
- Deliver medications consistent with the written care plan, physician orders, and established procedures, assessing their effectiveness and documenting all patient medications.
- Assist RN/RPNs and physicians in performing various examinations, treatments, and diagnostic procedures, such as physical examinations.
- Work collaboratively with the treatment team to assist patients with their program requirements, monitoring program requirements and motivational interviewing.
- Ensure infection control protocols are followed by all patients and personnel, observing behaviors, handling, storing, and disposing of workplace hazardous materials and sharps in accordance with established procedures.
- Perform regular security functions to maintain the safety and security of patients, staff, families, and the general public, taking immediate action to mitigate risks and reporting all real and potential risk issues.
- Assess individual patient risk factors and incorporate risk mitigation strategies into patients' care plans to optimize safety and security for staff, other inmates, and the general public.
- Graduation from a recognized program for Practical Nurses, plus 1 year of experience working with offenders and/or in secure healthcare settings, or an equivalent combination of education, training, and experience.
- Current full practicing licensure with the British Columbia College of Nursing Professionals (BCCNP).
- Occupational First Aid Level 2 certification (OFA Level 3 certification for positions at the Ford Mountain Correctional Centre).
