Community Health Support Specialist

Position Overview:
Aligned with the Vision, Purpose, and Values of Island Health, the safety and well-being of patients is a collective responsibility that everyone shares; thus, the commitment to enhancing quality and safety is integral to this role.
As part of a collaborative healthcare team, the Community Health Worker is responsible for delivering personal care and support to clients and their families, fostering and enhancing independence within the home environment. This includes assistance with daily living activities and executing delegated responsibilities as per established care plans.
Travel may be necessary for this role. Transportation arrangements must comply with the operational needs of Island Health and may necessitate the use of a personal vehicle, along with a valid driver's license.
Qualifications:Education, Training, and Experience
Completion of a Home Support/Resident Care Attendant/Health Care Assistant Certificate from an accredited post-secondary institution; or a suitable combination of education, training, and experience. A Food Safe Certificate and a valid driver's license are also required.
Current registration and good standing with the BC Care Aide & Community Health Worker Registry are essential.
Skills and Competencies- Ability to work autonomously with minimal supervision.
- Effective verbal and written communication skills.
- Proven ability to interact positively with others.
- Physically capable of fulfilling job responsibilities.
- Demonstrated organizational skills.
- Proficient in operating relevant equipment.
