Job Summary:
We are seeking a skilled and compassionate Youth Clinic Nurse to join our primary care team at Foundry's Granville Clinic under Providence Health Care. As a key member of our interdisciplinary team, you will provide high-quality nursing care to youth and young adults in a dynamic and supportive environment.
Key Responsibilities:
- Assess, plan, implement, and evaluate nursing care for patients/residents using clinical judgment and evidence-based practice.
- Develop and implement individualized care plans, incorporating patient/family learning needs and providing education and support.
- Collaborate with the interdisciplinary team to provide comprehensive care, including mental health and substance use support, sexual health care, and social services.
- Participate in nursing orientation and share clinical knowledge with other nursing staff, nursing students, and members of the interdisciplinary team.
- Support nursing research and quality improvement initiatives, and participate in program and quality improvement activities.
Requirements:
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives.
- Two years' recent, related experience working with youth and young adults in a primary care and mental health/substance use setting.
- Completion of the Point of Care HIV Testing course and successful completion of the BCCDC Immunization certificate.
- STI Management Certified Practice (or evidence of enrollment) required.
