Position Overview

We are seeking a compassionate and dedicated Support Transition Services Attendant to join our team in Sarnia-Lambton. This casual position offers an opportunity to make a meaningful difference in the lives of elderly individuals transitioning from hospital care to their homes.

Responsibilities

  • Provide personalized support to program participants during their transition home, adhering to established service plans.
  • Perform essential household tasks outlined in the service plan, including meal preparation, food safety, and maintaining a comfortable and secure environment for participants.
  • Conduct thorough risk assessments of participants' homes using standardized tools.
  • Facilitate access to community support services and guide participants through hospital discharge procedures, adhering to program guidelines.
  • Identify and report any potential hazards or equipment malfunctions, ensuring safe handling and storage of hazardous materials according to WHMIS guidelines.
  • Offer companionship to participants while maintaining a therapeutic distance.
  • Communicate any concerns regarding service recipients or their environment to the program coordinator.
  • Maintain strict confidentiality regarding participant information, caregivers, and the Canadian Red Cross.

Qualifications

  • Minimum certification in general first aid and CPR Level C is required.
  • Proficiency in English is essential; knowledge of French is an asset.
  • Experience working with elderly individuals is highly advantageous.
  • Strong interpersonal skills, reliability, punctuality, stress management abilities, and the capacity to build positive relationships are crucial.
  • Ability to follow care plans determined by relevant funding bodies.
  • Demonstrated autonomy and ability to thrive in an environment of uncertainty.
  • Excellent analytical and problem-solving skills; collaborative and productive team player.
