Transitional Support Services Coordinator

Job Summary:
The Canadian Red Cross is seeking a Transitional Support Services Worker to support clients during the transition from hospital to their home. As a key member of our team, you will work closely with the Program Manager to ensure a smooth transition process.
Key Responsibilities:
- Provide support to clients during the hospital discharge process and transition to their home, in accordance with established service plans.
- Complete basic errands and home management duties as required, including preparing nutritious meals and maintaining a safe and comfortable environment.
- Conduct risk assessments of clients' homes using standardized tools.
- Facilitate client and caregiver engagement, referrals to community support services, and discharge processes as per program guidelines.
- Identify and report any safety concerns or hazards, and handle and store hazardous products safely.
- Provide companionship to clients within therapeutic boundaries.
- Maintain confidentiality of clients, caregivers, and the Canadian Red Cross Society.
Requirements:
- Standard First Aid and CPR-C certification.
- Fluency in English is required, French is an asset.
- Previous experience working with elderly clients is an asset.
- Interpersonal skills, including dependability, punctuality, and ability to manage stress.
- Ability to follow care plans as identified by the appropriate funder agency.
- Self-starter with analytical and problem-solving skills, and ability to work in an environment with ambiguity.
Working Conditions:
- Duties are primarily conducted in clients' residences.
- Hours of operation are from 8:30 am to 4:30 pm, Monday to Friday.
- Irregular hours may be required, but never outside of regular operating hours.
