This critical role plays a vital part in enhancing the lives of our residents and their families, fostering a warm and welcoming environment that ignites human connection.

As a Resident & Family Experience Coordinator III, your key responsibilities will include:

Key Responsibilities:

  • Leading the resident move-in, transfer, and move-out process, ensuring seamless transitions and exceptional experiences.
  • Providing social work counseling services, collaborating with the inter-professional team to assess and develop plans of care that meet residents' psychosocial and social needs.
  • Planning, developing, implementing, and evaluating individual social work services and support groups to address the unique needs of residents and families.
  • Supporting residents and families during advanced care planning and treatment plan conversations, including funeral arrangement discussions.
  • Recruiting and supervising social work students' placements.
  • Responding to feedback and correspondence in a timely manner, ensuring satisfactory outcomes and maintaining a high level of customer satisfaction.

Requirements:

  • Social Worker registration per provincial legislation.
  • Minimum of three years' experience in healthcare, social services, or a related sector.
  • Knowledge of the Long Term Care Act & Regulations preferred.
  • Strong oral and written communication skills, with the ability to organize, plan, and delegate responsibilities.
