Job Title: Registered Nurse or Registered Practical Nurse
Job Summary:
Closing the Gap Healthcare Group is seeking a skilled and compassionate Registered Nurse or Registered Practical Nurse to join our team. As a key member of our healthcare team, you will provide high-quality patient care and support to our clients in their homes and communities.
Key Responsibilities:
- Provide direct patient care, including wound management, IV antibiotic administration, and other medical procedures.
- Conduct thorough assessments and develop individualized care plans to meet the unique needs of each client.
- Collaborate with healthcare professionals, including physicians and other nurses, to ensure seamless care coordination.
- Communicate effectively with clients, families, and caregivers to ensure understanding and compliance with care plans.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
- Current certification as a Registered Nurse (RN) or Registered Practical Nurse (RPN) with the College of Nurses of Ontario (CNO).
- Minimum of two years' experience in community healthcare, preferably with pediatric clients.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a strong focus on patient-centered care.
- Current certification in Standard First Aid & CPR/AED (Level C) and Mask Fit test.
