The Southlake Regional Health Centre is seeking a highly skilled and experienced Nurse Practitioner to join our Palliative Care team. As a key member of our multidisciplinary team, you will work collaboratively with physicians, nurses, and other healthcare professionals to provide high-quality patient care.
Key Responsibilities- Assess patients and families through comprehensive admission history and interview, advanced physical examination, and interpretation of diagnostic testing to determine need for medical, nursing, or allied health care.
- Communicate significant clinical findings to the attending physician and facilitate ongoing communication of patient progress and plan of care to the healthcare team.
- Initiate, order, tailor, and evaluate/discontinue selected medical and nursing therapy within the specialty scope of practice and consistent with practice guidelines and established medical directives for APN/ACNP to optimize the plan of care.
- Facilitate patient care by consulting with the patient/family and multidisciplinary team to formulate, initiate, coordinate, and evaluate a comprehensive and collaborative patient-centered plan of care, ensuring effective and efficient use of resources.
- Perform specialty-specific diagnostic and therapeutic interventions in accordance with scope of practice, approved medical directives for controlled acts, and established practice guidelines.
- Teach patients/families with focus on pain and symptom management, goals of care, palliative and end-of-life care, and caregiver support.
- Facilitate communication and collaboration among members of the healthcare team and refer to appropriate members of the team.
- Write/dictate admissions, consultations, progress notes, and discharge summary reports.
- Evaluate patient care outcomes and nursing practice issues to ensure the highest possible quality of care.
- Completion of patient treatment plans as required.
- Providing leadership to educational activities/programs related to Palliative Care.
- Facilitating and providing educational opportunities and programs, in collaboration with the Nurse Educator, for nursing, medical, and allied health students and staff for the enhancement of specialized knowledge and skills related to the specialty area.
- Mentor/Preceptor of students (graduate and undergraduate) and other interdisciplinary students as required in their clinical experience with evaluation of performance.
- Engaging in joint activities with Faculties of Nursing, and other academic faculties and organizations through collaborative projects, research, and cross appointments.
- The development, implementation, and evaluation of quality management programs/initiatives, care delivery models, practice standards/guidelines/policies, and procedures, and associated health outcomes and resource utilization, in collaboration with the leadership teams.
- Identify patterns and trends in healthcare and patient care outcomes and develop alternate approaches in relation to patient care situations.
- Participate on committees relevant to patient care issues.
- Promote advanced nursing practice in program planning, policy making, and problem solving.
- Registered Nurse and in good standing with the College of Nurses of Ontario (CNO) required.
- Master in Nursing/Master of Science in Nursing with successful completion of a Nurse Practitioner Program from a recognized school of graduate studies required.
- Current College of Nurses of Ontario registration in the Extended Class (EC) as an NP (adult) or NP (primary health care) required.
- Pallium LEAP certification or equivalent required.
- Completion of ongoing education in Palliative Care such as the DeSouza Symptom Assessment and Management in Palliative Care required.
- Pallium LEAP Facilitator Certification preferred.
- Recent (within the last 12 months) certification in chemotherapy administration required, demonstration of annual recertification from a recognized institution (de Souza Institute/Oncology Nurses Society [ONS])
- Basic Cardio Life Saving (BCLS) required.
- CON (C) or equivalent preferred.
- Additional specialty education in Oncology preferred i.e. de Souza Hematology Courses.
- Active member of Canadian Association of Nurses in Oncology (CANO) preferred.
- Minimum 5 years of recent and relevant Registered Nurse experience in oncology/palliative care required.
- Minimum 3 years of recent and relevant Nurse Practitioner experience in oncology preferred.
- Experience with medical oncology and radiation oncology protocols, prescribing of pain management and palliative supportive therapies required.
- Demonstrated strong communication and interpersonal skills, with the ability to work collaboratively with physicians, inter-professional team members, and other stakeholders.
- Possesses highly developed assessment, analytical, problem-solving, and critical thinking skills.
- Excellent oral, written, and presentation skills, with demonstrated competency in information and communication technology.
- Demonstrated excellent organizational and coordination skills and the ability to prioritize multiple competing workload demands.
- Demonstrated ability to provide leadership, support, and critical analysis in stressful, complex clinical and emotional situations.
- Demonstrated commitment to customer service.
- Demonstrated individual leadership skills.
- Demonstrated experience in adult education an asset.
