Position Summary

The Nurse Practitioner plays a pivotal leadership role within a multidisciplinary and inter-professional team. Collaborating closely with Medical Oncologists, Haematologists, and other healthcare professionals, the Nurse Practitioner addresses the needs of patients with solid tumours, malignant haematology, and oncology, particularly within the Outpatient SRCC Solid Tumour and Malignant Haematology sectors. This role emphasizes partnership with all relevant stakeholders, excelling in the four domains of practice: clinical care, leadership, education, and research.

Key Responsibilities

  • Employs diagnostic reasoning and decision-making processes to develop hypotheses, determine the necessity for, order, and interpret diagnostic tests, and establish medical diagnoses within the specialty scope of practice, adhering to established guidelines.
  • Conducts thorough assessments of patients and families through various methods, including comprehensive admission histories, advanced physical examinations, interval assessments, and diagnostic test interpretations to ascertain the need for medical, nursing, or allied health interventions.
  • Communicates significant clinical findings to the attending physician and ensures ongoing communication regarding patient progress and care plans with the healthcare team.
  • Initiates, orders, customizes, and evaluates or discontinues selected medical and nursing therapies (including pharmacologic treatments) within the specialty scope of practice, in alignment with established medical directives.
  • Facilitates patient care by collaborating with patients, families, and the multidisciplinary team to develop, initiate, coordinate, and assess a comprehensive, patient-centered care plan, ensuring optimal resource utilization.
  • Performs specialty-specific diagnostic and therapeutic interventions in accordance with approved medical directives and established practice guidelines.
  • Educates patients and families on pain and symptom management, goals of care, palliative and end-of-life care, and caregiver support.
  • Promotes communication and collaboration among healthcare team members and refers patients to appropriate team members as necessary.
  • Documents admissions, consultations, progress notes, and discharge summaries accurately.
  • Evaluates patient care outcomes and nursing practice issues to maintain the highest quality of care.
  • Completes patient treatment plans as required.
  • Leads educational activities and programs related to Palliative Care.
  • Facilitates educational opportunities and programs in collaboration with the Nurse Educator for nursing, medical, and allied health students and staff to enhance specialized knowledge and skills.
  • Mentors and precepts students (graduate and undergraduate) and other interdisciplinary students during their clinical experiences, evaluating their performance.
  • Engages in collaborative projects, research, and cross-appointments with academic faculties and organizations.
  • Develops, implements, and evaluates quality management programs, care delivery models, practice standards, guidelines, policies, and procedures, in partnership with leadership teams.
  • Identifies patterns and trends in healthcare and patient outcomes, developing alternative approaches to patient care situations.
  • Participates in committees addressing patient care issues.
  • Advocates for advanced nursing practice in program planning, policy-making, and problem-solving.
  • Performs additional duties consistent with the job classification as required.

Qualifications

  • Registered Nurse in good standing with the College of Nurses of Ontario (CNO) required.
  • Master's degree in Nursing or Master of Science in Nursing with successful completion of a Nurse Practitioner Program from an accredited institution required.
  • Current registration with the College of Nurses of Ontario in the Extended Class (EC) as an NP (adult) or NP (primary health care) required.
  • Pallium LEAP certification or equivalent required.
  • Completion of ongoing education in Palliative Care, such as the DeSouza Symptom Assessment and Management in Palliative Care, required.
  • Pallium LEAP Facilitator Certification preferred.
  • Recent certification in chemotherapy administration required, with evidence of annual recertification from a recognized institution.
  • Basic Cardio Life Saving (BCLS) certification required.
  • CON (C) or equivalent preferred.
  • Additional specialty education in Oncology preferred.
  • Active membership in the Canadian Association of Nurses in Oncology (CANO) preferred.
  • A minimum of 5 years of recent and relevant Registered Nurse experience in oncology/palliative care required.
  • A minimum of 3 years of recent and relevant Nurse Practitioner experience in oncology preferred.
  • Experience with medical oncology and radiation oncology protocols, as well as prescribing pain management and palliative supportive therapies, required.
  • Demonstrated strong communication and interpersonal skills, with the ability to collaborate effectively with physicians and inter-professional team members.
  • Possesses highly developed assessment, analytical, problem-solving, and critical thinking skills.
  • Excellent oral, written, and presentation skills, with demonstrated competency in information and communication technology.
  • Demonstrated organizational and coordination skills, with the ability to prioritize multiple competing workload demands.
  • Demonstrated ability to provide leadership and support in complex clinical and emotional situations.
