Job Title: Community Health Nurse

We are seeking a highly skilled and compassionate Community Health Nurse to join our team at Fraser Health Authority. As a Community Health Nurse, you will play a vital role in providing high-quality patient care and promoting health and well-being in our community.

Job Summary

The Community Health Nurse will work independently in the community setting, assessing and treating complex geriatric and adult patients with chronic health conditions. You will collaborate with an interprofessional team to develop and implement comprehensive care plans, ensuring seamless transitions across the healthcare continuum.

Responsibilities
  • Conduct thorough assessments and develop individualized care plans for clients with complex health needs.
  • Provide direct client care, including education, counseling, and support to promote health and well-being.
  • Collaborate with healthcare professionals, community resources, and family members to ensure comprehensive care and support.
  • Develop and maintain accurate and detailed clinical records, including intake screens, client assessments, and progress notes.
  • Participate in quality improvement initiatives and contribute to the development of standards of care, policies, and procedures.
Requirements
  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • One (1) year recent related clinical experience assessing and treating complex geriatric and/or adult patients with chronic health conditions.
  • Valid BC Driver's license and access to a personal vehicle for business-related purposes.
