Job Title: Community Health Nurse

We are seeking a skilled Community Health Nurse to join our team at Fraser Health Authority. As a Community Health Nurse, you will play a vital role in providing high-quality patient care and promoting health and wellness in our community.

Job Summary:

The successful candidate will be responsible for assessing and treating patients with complex geriatric and adult health conditions in an acute or community/outpatient care setting. You will work collaboratively with an interprofessional team to develop and implement comprehensive care plans, provide education and support to patients and families, and facilitate transitions across the healthcare continuum.

Responsibilities:
  1. Conduct initial and ongoing client assessments to identify health needs and develop individualized care plans.
  2. Provide nursing care through therapeutic interventions, including education, counseling, and direct client care.
  3. Collaborate with the interprofessional team to coordinate care and services, including referrals to other healthcare providers and community resources.
  4. Develop and implement care plans that promote client autonomy, dignity, and quality of life.
  5. Provide health education and support to patients and families to promote health and wellness.
  6. Facilitate transitions across the healthcare continuum, including discharge and transfer to other services.
  7. Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Qualifications:

Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM). One (1) year's recent related clinical experience assessing and treating complex geriatric and/or adult patients with chronic health conditions in an acute or community/outpatient care setting, including recent experience in care and discharge planning. Valid BC Driver's license and access to a personal vehicle for business-related purposes.
