About the RoleThe Community Health Registered Nurse will be responsible for assessing and treating complex geriatric and adult patients with chronic health conditions, providing care and discharge planning, and promoting client-focused care. The successful candidate will have a strong understanding of gerontology and adults living with complex frailty and chronic illnesses.
Key Responsibilities- Establishes a therapeutic relationship with clients through interpersonal and interviewing techniques
- Provides individualized client assessments, interprofessional care planning, and interventions
- Develops and implements comprehensive shared client care plans in collaboration with the interprofessional team and primary care provider
- Provides direct client care and identifies other care services required in accordance with applicable guidelines and policies
- Collaborates with other healthcare professionals to facilitate the flow of information and coordinate the integration of care and services
- Maintains clinical records and statistical information on patient/client care
- Participates in department quality improvement and risk management activities
- Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives
- One year's recent related clinical experience assessing and treating complex geriatric and/or adult patients with chronic health conditions
- Valid BC Driver's license and access to a personal vehicle for business-related purposes
