Job Title: Community Health Nurse

Fraser Health is seeking a skilled Community Health Nurse to join our team. As a Community Health Nurse, you will work independently in the community setting, collaborating with an interprofessional team to manage client caseloads and provide care services.

Key Responsibilities:
  • Assess and treat complex geriatric and adult patients with chronic health conditions
  • Develop and implement care plans, including referrals to other services and interventions
  • Collaborate with primary care providers, clients, and families to ensure seamless care transitions
  • Provide health counseling, education, and self-management support to clients
  • Maintain accurate clinical records and statistical information
Requirements:
  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives
  • One year recent related clinical experience in acute or community/outpatient care settings
  • Valid BC Driver's license and access to a personal vehicle for business-related purposes
