Job SummaryWe are seeking a highly motivated and experienced Occupational Therapist to provide high-quality patient care, coordinate with Community Support Workers, and develop effective Care Plans.
Key Responsibilities:- Conduct thorough assessments of patients in the community and in their homes to identify rehabilitation needs.
- Develop and maintain effective Care Plans in collaboration with the care team, focusing on patient-centered goals and outcomes.
- Work closely with patients to establish and achieve measurable goals, ensuring timely and effective progress.
- Provide guidance on workplace Health and Safety protocols.
- Perform other duties as required to support the care team.
- Master's Degree in Occupational Therapy from a recognized university.
- Registration with the College of Occupational Therapists of Ontario.
- Proven computer skills, including email, MS Word, and Outlook, work scheduling systems.
- Strong assessment skills and ability to participate in effective treatment programs.
- Excellent English verbal and written communication skills.
- French language skills are an asset.
