The University Health Network is seeking a skilled Clinical Social Worker to join our General Internal Medicine team. As a key member of our interprofessional team, you will work closely with patients, families, and healthcare providers to provide comprehensive social work services.

  • Assess patients' social and emotional needs, and develop individualized plans to address these needs.
  • Collaborate with healthcare providers to ensure seamless care coordination and communication.
  • Provide education and support to patients and families on health-related issues and community resources.
  • Participate in research activities and quality improvement initiatives to enhance patient care and outcomes.
  • Develop and maintain relationships with community agencies and organizations to ensure access to resources and services for patients.

  • Master's degree in Social Work (MSW) or equivalent.
  • Registration with the Ontario College of Social Workers and Social Service Workers.
  • Membership in the Ontario Association of Social Workers, preferred.
  • 2-3 years of experience working in a healthcare setting, preferably in an interprofessional team.
  • Knowledge of policies, legislation, and programs related to social welfare and healthcare.
  • Research experience and education, an asset.
  • Student education and supervision, an asset.
  • Knowledge of a second language, an asset.
