About the Role

We are seeking a highly skilled Social Worker to join our team at the University Health Network, where you will play a critical role in providing exceptional care to seniors in our Emergency Medicine department.

As a Social Worker in this position, you will work collaboratively with our interprofessional team to assess patients' needs, develop and implement intervention plans, and provide clinical teaching to students and colleagues.

With a strong focus on quality improvement and patient-centered care, you will contribute to the development and implementation of a new model of care that addresses the complex needs of our aging population.

Key Responsibilities
  • Assess patients' needs and develop individualized intervention plans
  • Collaborate with the interprofessional team to provide comprehensive care
  • Provide clinical teaching and supervision to students and colleagues
  • Contribute to quality improvement initiatives and the development of new care models
  • Liaise with community agencies and healthcare providers to ensure seamless care transitions
Requirements
  • Master's degree in Social Work or equivalent
  • Registration with the Ontario College of Social Workers and Social Service Workers
  • Membership in the Ontario Association of Social Workers (optional)
  • 2-3 years of experience working in a hospital setting, preferably in Emergency Medicine or Acute Care
  • Excellent communication, interpersonal, and problem-solving skills
  • Knowledge of policies, legislation, and programs related to social welfare institutions and services
