About the RoleThis is a temporary full-time position that will provide social work services to patients and their families in a hospital setting. The successful candidate will work collaboratively with an interprofessional team to assess patients' needs, develop and implement treatment plans, and provide education and support to patients and their families.
Key Responsibilities- Assess patients' needs and develop treatment plans in collaboration with the interprofessional team
- Provide education and support to patients and their families to promote health and well-being
- Facilitate family/team meetings to promote communication and collaboration
- Liaise with community agencies to provide patients with access to community resources
- Participate in research activities and projects to improve social work practice and patient outcomes
- Master's degree in Social Work (M.S.W) or equivalent
- Registration with the Ontario College of Social Workers and Social Service Workers
- Membership in the Ontario Association of Social Workers, preferred
- 2-3 years experience working with an interprofessional team in a healthcare setting, preferred
- Knowledge of policies, legislation, programs, and issues related to social welfare institutions and services
