Primary Objective: The Project Manager for the initiative titled, "Saskatchewan Oncofertility Algorithm (SOFA): Crafting an Inclusive Pathway for Pediatric, Adolescent, and Young Adult Patients", is responsible for the coordination and management of provincial efforts aimed at enhancing patient experiences through the establishment of an oncofertility pathway in Saskatchewan. This role involves close collaboration with fertility specialists, pediatric and medical oncologists, and various healthcare professionals who cater to Adolescent Young Adult (AYA) patients in the region. The focus will be on developing a standardized oncofertility referral pathway and resources tailored for AYA patients, particularly addressing the needs of First Nations and Métis communities, as well as those from rural and remote areas requiring oncofertility assistance. The individual in this position will oversee the daily operations necessary to achieve all objectives related to the AYA Oncofertility project.

Key Responsibilities:

  • Establishes an AYA working group to produce patient-facing resources related to oncofertility.
  • Forms an AYA-Health Care Professional (HCP) steering committee to play a crucial role in the design of the referral pathway and to provide input on educational resources for healthcare teams and patients/families.
  • Develops a practical, equitable, and sustainable fertility referral pathway for both pediatric and adult cancer patients.
  • Maintains a strong commitment to creating a culturally safe and accessible oncofertility pathway for First Nations and Métis patients.
  • Integrates a sustainability plan to embed AYA Oncofertility resources, discussions, and referral pathways into clinical practice and culture.
  • Prepares reports and documentation for improvement, including process descriptions, interconnected quality initiatives, limitations, gaps, risks, and recommended mitigation strategies.
  • Collaborates with provincial and site-based staff educators to support and enhance staff education strategies related to the utilization of AYA Oncofertility resources and referral pathways.

Partnership and Collaboration

  • Maintains and develops stakeholder relationships essential for supporting AYA Oncofertility initiatives within the Saskatchewan Cancer Agency and Pediatric Oncology Service, particularly in collaboration with provincial fertility services and members of First Nations and Métis communities.
  • Works closely with the AYA-HCP Steering Committee to guide AYA Oncofertility efforts in Saskatchewan and engage with other stakeholders to inform the work.
