Position Overview

We are seeking a dedicated Registered Nurse specializing in Primary Care

Vancouver Coastal Health is in search of a Registered Nurse to become a vital part of the City Centre Urgent and Primary Care Centre (UPCC) team. This role is essential in delivering quality care to our community.

Key Responsibilities as a Primary Care Nurse:

  • Conduct triage, assessments, planning, implementation, and evaluation of nursing care for clients and families requiring immediate attention.
  • Foster and sustain collaborative relationships with primary care providers aligned with the local Community of Care (COC) service delivery model and primary care network.
  • Utilize established triage standards while applying critical thinking skills within the safe parameters of patient care.
  • Implement chronic disease management, palliative care, health management, and self-care management principles to perform assessments, identify issues, provide direct care, and address any deviations from the client's care plan.
  • Serve as a resource for the interdisciplinary team by sharing knowledge based on nursing theory and practice related to patients with primary care needs, adhering to the standards set by the British Columbia College of Nurses and Midwives (BCCNM) and VCH policies.

About Urgent and Primary Care Centres:

Urgent and Primary Care Centres (UPCC) are designed to offer care for individuals experiencing unexpected, non-life-threatening conditions requiring medical attention within a short timeframe. This includes minor injuries, infections, and less severe health issues. The multidisciplinary healthcare team comprises general practitioners, nurse practitioners, registered nurses, and social workers, working together to provide timely care.

UPCCs serve as an additional resource in the community, coordinating with various care points to ensure patients receive appropriate urgent services when needed.

Qualifications

Education & Experience

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • A minimum of two (2) years of recent clinical experience relevant to the designated clinical area, or an equivalent combination of education, training, and experience.

Knowledge, Skills & Abilities

  • Proven ability to apply critical thinking skills within the safe limits of patient care.
  • In-depth knowledge and skills in primary care and a patient and family-centered care model.
  • Familiarity with relevant Professional Regulations and Standards, including Community Health Nurse of Canada (CHNC) competencies.
  • Expertise in evidence-based clinical practices related to primary care.
  • Understanding of provincial acts, regulations, and program policies related to home, community, and primary care.
  • Knowledge of acute and chronic diseases, mental health, substance use, palliative care, and self-management support.
  • Ability to provide culturally safe care to diverse populations.
  • Experience in Trauma Informed Practice, Harm Reduction, and Recovery Oriented Care.
  • Strong communication skills.
  • Effective conflict resolution abilities.
  • Capacity to work independently and prioritize care effectively.
  • Understanding of other health disciplines and their roles in patient care.
  • Ability to integrate research into practice.
  • Knowledge of adult education principles and the ability to educate and support patients and families.
  • Proficiency in CPR techniques.
  • Physical capability to perform the duties of the position.
  • Competence in operating relevant equipment.
