About the Opportunity

Nova Scotia Health is seeking a skilled professional to fill the role of Challenging Behavior Coordinator. As a key member of our team, you will play a vital part in enhancing the capacity of continuing care providers to deliver high-quality care to older adults with complex cognitive and mental health needs.

About You
  • We are looking for a candidate with a university degree in a regulated healthcare profession and a minimum of 5 years of experience in the field.
  • A strong background in geriatrics, mental health, and/or ABI populations is essential.
  • Excellent communication and interpersonal skills are required to work effectively with clients, families, and healthcare teams.
  • Experience in developing and delivering education programs and facilitating knowledge transfer is an asset.
  • Knowledge of community agencies, service resources, and outreach teams is also an asset.
Responsibilities
  • Facilitate and support the implementation of the Behavioral Health Program with continuing care providers.
  • Provide guidance and advice to promote the coordination and integration of services.
  • Work collaboratively with healthcare teams to develop and implement care plans.
  • Conduct assessments and provide recommendations for clients with responsive behavior.
  • Develop and deliver education programs to enhance the capacity of continuing care providers.
Requirements
  • Valid Nova Scotia driver's license and access to a reliable vehicle.
  • Ability to work under pressure and manage multiple tasks simultaneously.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and manage sensitive issues with tact and diplomacy.
