About the Opportunity

As a key member of the Continuing Care Behavioral Health team, the Challenging Behavior Resource Coordinator will play a vital role in facilitating the implementation of the Behavioral Health Program with the continuing care sector. This includes enhancing the capacity of continuing care providers to provide care to older persons with complex cognitive and mental health needs. The successful candidate will provide leadership and collaboration to promote the coordination and integration of services, ensuring the highest level of patient care and satisfaction.

About You

We are seeking a highly skilled and experienced professional with a strong background in a regulated healthcare profession. The ideal candidate will have a minimum of 5 years' experience within the profession, be a member in good standing with a college or regulatory governing body in Nova Scotia, and possess a strong understanding of community agencies, service resources, and outreach teams. Excellent communication and interpersonal skills are essential for this role, as is the ability to work autonomously and collaboratively with other professionals and para-professionals.

Key Responsibilities
  • Facilitate the implementation of the Behavioral Health Program with the continuing care sector
  • Enhance the capacity of continuing care providers to provide care to older persons with complex cognitive and mental health needs
  • Provide leadership and collaboration to promote the coordination and integration of services
  • Develop and deliver education programs and facilitate knowledge transfer with both individuals and teams
  • Work collaboratively with other professionals and para-professionals to achieve the highest level of patient care and satisfaction
Requirements
  • Completion of a university degree in a regulated healthcare profession
  • Minimum of 5 years' experience within the profession
  • Member in good standing with a college or regulatory governing body in Nova Scotia
  • Strong understanding of community agencies, service resources, and outreach teams
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and collaboratively with other professionals and para-professionals
