Job Summary:
We are seeking a skilled Registered Nurse to join our team at Trillium Health Partners. As a Nurse Specialist, you will play a critical role in supporting patients within the Clozaril Clinic, working in partnership with treatment teams to provide high-quality care.
Key Responsibilities:
- Support patients with complex psychiatric disorders, including those with concurrent disorders.
- Complete blood draws and collection, monitor lab values, and communicate with patients and treatment teams as needed.
- Collaborate with community agencies, including laboratory facilities and pharmacies, to ensure seamless coordination of patient care.
- Provide ongoing education and support to patients and families, and monitor for and process referrals to the Clozaril clinic.
- Work effectively within an inter-professional collaboration model, utilizing exceptional organizational, time management, and critical thinking skills.
Requirements:
- Registered Nurse with current registration with the College of Nurses of Ontario and in good standing.
- Graduate of a recognized College or School of Nursing, with Baccalaureate degree preferred.
- CNA certification in psychiatric/mental health nursing preferred.
- Minimum 5 years current Mental Health experience in hospital-based setting.
- Sound working knowledge of legislation related to mental health care and treatment.
- Expertise and experience in venipuncture, knowledge of all relevant medications and side effects.
- Knowledge and experience related to concurrent disorders, case co-ordination, and counselling.
- Knowledge of Recovery Model, and proven ability to interact with patients and their families in an empathetic manner.
