Job Summary
We are seeking a highly skilled and compassionate Client Care Coordinator to join our team. As a Client Care Coordinator, you will play a vital role in providing administrative support to our healthcare professionals and ensuring the smooth delivery of our services.
Key Responsibilities
- Provide administrative support to healthcare professionals, including scheduling and coordination of services
- Liaise with clients, staff, and community partners to ensure seamless communication and collaboration
- Act as a problem solver for clients and respond to inquiries from healthcare professionals and employees
- Report client and staff concerns to the Home Support Supervisor
- Liaise with HCCSS representatives regarding client care
- Collect and enter all pertinent client data into the computer system
- Provide lists, schedules, and perform other administrative duties to support the personal support worker and Nurse program
Requirements
- Minimum of 2 years working in an office setting
- Ability to work weekends
- Ability to handle a high volume of calls while delivering exceptional customer service
- Ability to multitask and prioritize
- High school diploma or equivalent
- Excellent English written and verbal communication skills
- Excellent customer service and organizational skills
- Proficient in Word, Outlook, and ability to learn proprietary software
