Job Summary

We are seeking a highly skilled and compassionate Client Care Coordinator to join our team. As a Client Care Coordinator, you will play a vital role in providing administrative support to our healthcare professionals and ensuring the smooth delivery of our services.

Key Responsibilities

  • Provide administrative support to healthcare professionals, including scheduling and coordination of services
  • Liaise with clients, staff, and community partners to ensure seamless communication and collaboration
  • Act as a problem solver for clients and respond to inquiries from healthcare professionals and employees
  • Report client and staff concerns to the Home Support Supervisor
  • Liaise with HCCSS representatives regarding client care
  • Collect and enter all pertinent client data into the computer system
  • Provide lists, schedules, and perform other administrative duties to support the personal support worker and Nurse program

Requirements

  • Minimum of 2 years working in an office setting
  • Ability to work weekends
  • Ability to handle a high volume of calls while delivering exceptional customer service
  • Ability to multitask and prioritize
  • High school diploma or equivalent
  • Excellent English written and verbal communication skills
  • Excellent customer service and organizational skills
  • Proficient in Word, Outlook, and ability to learn proprietary software
