In the role of Bilingual Patient Care Coordinator, you will evaluate and identify patient care requirements and eligibility, facilitate access to community resources, and collaborate with patients, their families, and other healthcare professionals.

Whether you are working from an office, within the community, or in a healthcare facility, you will be instrumental in delivering integrated, accessible, and patient-focused care. You will be supported by a collaborative team comprising over 9,000 regulated health professionals.

Key Responsibilities:

Conduct thorough patient assessments utilizing validated tools to evaluate clinical, physical, and psychosocial needs, including those of caregivers. Assess the understanding and educational needs of patients and caregivers regarding diagnoses, treatment options, available resources, and coping strategies. Support patients and caregivers in establishing realistic goals, whether individual, joint, short-term, or long-term. Collaborate with patients, caregivers, and the care team to create a personalized care and service plan that reflects the patient's priorities and desired outcomes, identifying necessary strategies and resources. Coordinate the provision of home and community care services for patients and their caregivers. Manage vendor and resource utilization for medical equipment, supplies, and services. Facilitate effective communication and collaboration among the multidisciplinary healthcare team to optimize patient outcomes. Initiate and authorize the execution of care plans through contracted service providers. Provide regular updates to physicians, service providers, patients, caregivers, and other authorized individuals as necessary. Maintain case documentation in accordance with established policies.

Qualifications:

A valid registration as a Nurse, Physiotherapist, Occupational Therapist, Social Worker, Speech-Language Pathologist, or Dietitian in Ontario. Relevant professional experience, preferably in community settings, typically acquired over two years. Compliance with mandatory COVID-19 vaccination policies is required.

Preferred Skills:

Experience working with diverse patient populations with varying comprehension and language abilities. Proficient in managing challenging conversations and situations, such as advance care planning. Ability to work effectively both independently and as part of a team. Strong verbal and written communication skills to build relationships with a variety of stakeholders within the healthcare community. Excellent listening and facilitation skills to engage with patients and their caregivers. Diplomatic approach to sensitive and confidential matters. Proven relationship-building skills to foster partnerships with health and community stakeholders. Proficient in using computer software, including Microsoft Office applications. A valid driver's license and access to a vehicle are required. Fluency in both English and French is essential. A recent Vulnerable Sector Check is required.

Work Schedule

The position involves a standard workweek of Monday to Friday, from 8:30 AM to 4:30 PM, totaling 35 hours per week. Flexible work options are available, including a hybrid model of in-office and remote work.
