Job Summary:

We are seeking a skilled and compassionate nurse to provide direct patient care in an acute medical unit. As a Registered Nurse with Fraser Health, you will work collaboratively with the interprofessional care team to provide person-centered care to patients.

Key Responsibilities:
  • Assess patients and collect information from various sources to make informed nursing diagnoses and develop effective care plans.
  • Collaborate with patients and their families/care givers to develop and implement care plans that promote autonomy and dignity.
  • Participate in the care and discharge planning process for patients, ensuring comprehensive care planning and delivery.
  • Coordinate nursing care for patients, prioritizing tasks and delegating responsibilities to support staff.
  • Collaborate with the interprofessional care team to identify and resolve issues, adjust care plans as needed, and address changes in patient condition.
  • Teach patients and families/care givers about illness prevention, health restoration, and health maintenance to support self-care and discharge plans.
  • Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
Qualifications:
  • Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Completion of applicable post-graduate coursework or an equivalent combination of education, training, and experience.
  • Current certification in Basic Cardiac Life Support (BCLS).
Skills and Abilities:
  • Ability to complete initial and ongoing patient assessment and provide nursing care through appropriate/prescribed technical, therapeutic, safety-type interventions.
  • Ability to communicate effectively with patients, families/care givers, and members of the interprofessional care team.
  • Ability to promote person-centered care that demonstrates empathy, sensitivity to diverse cultures and preferences, and patient advocacy.
  • Ability to integrate and evaluate data from multiple sources to problem-solve effectively.
  • Ability to teach and instruct patients and families/care givers about topics essential to their health care and well-being.
  • Ability to organize and prioritize work.
  • Ability to collaborate with and promote cooperation among members of the interprofessional care team.
