Job Title: Case Manager - Early Psychosis Intervention

We are seeking a highly skilled and compassionate Case Manager to join our Early Psychosis Intervention team at the Canadian Mental Health Association. As a Case Manager, you will work collaboratively with clients, families, and healthcare professionals to provide individualized support and services to individuals experiencing early psychosis.

Key Responsibilities:
  • Assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support to clients
  • Collaborate with clients to provide individualized and flexible services to engage, support, and integrate them into the community
  • Facilitate clients' use of resources and supports to maximize their independence and potential
  • Participate in individual and systemic advocacy initiatives to ensure clients' wishes are heard and addressed
Requirements:
  • Degree with four years' experience or Diploma with six years' experience in a community-based mental health organization
  • Current registration with OCSWSSW or a regulatory body under the Regulated Health Professions Act
  • A second language reflecting the local community is an asset
  • Knowledge of the Ontario Mental Health Act, mental health reform principles, and relevant legislation
  • Ability to apply Recovery principles and empowerment-oriented philosophies and practices in work with consumers
