Interprofessional Primary Care Team

This role is integral to Baycrest's Ambulatory Services. As a member of the interprofessional primary care and transitional outreach teams, you will deliver primary care to clients aged 18 and older who live independently in the community. Your focus will be on addressing physical, cognitive, psychosocial, and environmental factors affecting their functionality, particularly in chronic disease management, frailty, mental health, and primary care.
You will provide health education aimed at helping clients achieve their health objectives and enhance their quality of life. Your assessments and goal-setting will inform the comprehensive treatment plans for clients. Collaboration with primary care physicians will be essential to support patient attachment to primary care and ensure collaborative care. This includes offering both individual and group health education.
Key Responsibilities:
- Deliver and manage primary care for clients without a Primary Care Provider, facilitating connections to Family Doctors.
- Conduct advanced assessments, formulate diagnoses, and develop therapeutic treatment plans.
- Ensure continuity of care through collaboration with interprofessional health teams, including family physicians and community partners.
- Communicate findings and health outcomes to patients, families, and care teams.
- Support the program and interdisciplinary team in developing innovative service delivery models.
- Advocate for clients at the point of care and contribute to health system policy discussions.
- Encourage clinical inquiry that fosters nursing research and enhances patient care.
- Identify system needs and recommend improvements for clinical practice and client outcomes.
- Participate in team meetings and evaluation activities.
- Some evenings and weekends may be required based on program needs.
Qualifications:
- Current certificate of competence from the College of Nurses of Ontario in the Extended Class (RN (EC)).
- Preferred membership in the Nurse Practitioners' Association of Ontario (NPAO).
- Nurse Practitioner-Primary Health Care (NP-PHC) or Nurse Practitioner-Adult (NP-Adult) certification from an accredited program.
- Competency in prescribing narcotics/controlled substances is required.
- National certification in Gerontology Nursing from the CNA.
- Minimum of five years of nursing experience in geriatric and primary care across various settings.
- Experience in primary care with diverse age groups is advantageous.
- Demonstrated ability in autonomous diagnosis, ordering, and interpreting diagnostic tests.
- Advanced computer and mobile communication skills are essential.
- Effective leadership skills, including flexibility and creativity.
- Strong interpersonal skills.
- Experience in research, program planning, implementation, and evaluation.
- Knowledge of the regional health system, particularly in geriatrics and mental health, is a plus.
