Sinai Health is seeking an experienced Nurse Practitioner to support our Medical/Neuro Complex Care and Reconditioning Service department.
Key Responsibilities- Conduct comprehensive assessments, formulate diagnoses, and develop individualized plans of care.
- Provide expert care for a diverse range of acute and chronic conditions, utilizing advanced clinical and theoretical knowledge.
- Collaborate with the interprofessional team to coordinate and deliver high-quality patient care.
- Utilize effective communication and counseling skills to support patient and family-centered care.
- Participate in quality improvement initiatives and contribute to the development of the interprofessional team.
- Provide leadership and promote excellence in patient-centered nursing care through expert practice, education, and consultation.
- Current College of Nurses of Ontario registration in the Extended Class (EC) as an NP-Adult or NP-Primary Health Care.
- Master's in Nursing/Nurse Practitioner (MN-NP) or Master's in Nursing with successful completion of a post-Master's Nurse Practitioner Certificate Program from a recognized school of graduate studies.
- 5 years of relevant acute care or post-acute experience in the inpatient setting (e.g., medicine, medical surgical nursing, intensive care, complex continuing care).
- 1-3 years of primary care experience an asset.
- Experience with complex wound care assessment and treatment an asset.
- Additional specialty geriatric education and experience with seniors and frail elderly preferred.
- Experience in caring for people with mental health and/or geriatric psychological conditions an asset.
- Current CPR certification.
- Demonstrated experience through roles of advanced practitioner, educator, researcher, and leader.
- Demonstrated strong communication and interpersonal skills.
- Highly developed analytical, problem-solving, and critical thinking skills.
- Excellent oral, written, and presentation skills, with demonstrated competency in information and communication technology.
