The Canadian Association of Community Health Centres is a non-profit organization dedicated to enhancing the health and well-being of communities through accessible primary healthcare and social services. Our commitment is to empower individuals facing barriers to their health, ensuring they receive equitable access to essential services and quality care through various evidence-based programs.

The HCV Support Specialist will be an integral part of our community-focused Hepatitis C program, which aims to enhance treatment access for individuals who use drugs and/or alcohol. This role involves providing essential support to program participants and facilitating program activities. The Specialist will collaborate with a multidisciplinary team to refine existing services and contribute to the creation of innovative initiatives.

Key Responsibilities:

  1. Distributing harm reduction supplies and providing education on harm reduction strategies.
  2. Conducting community outreach and delivering educational sessions on HCV.
  3. Co-facilitating group sessions and supervising drop-in services.
  4. Engaging in public speaking, educational outreach, and advocacy efforts.
  5. Offering one-on-one support to program participants, including accompanying them to appointments.
  6. Making referrals to relevant resources and services.
  7. Assisting in the preparation of group activities and special community events.
  8. Supporting participants in understanding testing results and managing their HCV treatment journey.
  9. Contributing to research, surveys, and program evaluations.
  10. Maintaining accurate records and documentation of services provided.
  11. Participating in meetings and training sessions.
  12. Attending regular supervisory and support meetings.
  13. Upholding confidentiality and maintaining professional boundaries.
  14. Serving as a positive role model for program participants.
  15. Performing additional duties as required.

Qualifications:

  1. Completion of a Peer training program or equivalent experience.
  2. Personal experience with HCV and/or HIV, as well as familiarity with substance use, homelessness, or mental health challenges.
  3. Respect for diversity, including sexual orientation, gender identity, race, ability, and life experiences.
  4. Strong sensitivity to the needs of others.
  5. Commitment to diversity, equity, anti-racism, and accessibility, with a collaborative approach.
  6. Ability to work effectively within a team environment.
  7. Excellent communication and active listening skills.

  17. Clearance documentation may be required for employment.
  18. Proof of vaccination status may be necessary for employment.
