Nurse Care Coordinator

The Clinical Care Specialist:
•Develops and implements comprehensive care plans to support clients in achieving their goals and improving their quality of life.
•Collaborates with healthcare providers to deliver high-quality, patient-centered care and ensures seamless transitions between services.
•Enhances the client's ability to manage their own care and navigate the community setting by assessing service eligibility and providing care management.
•Demonstrates continuous improvement and evidence-based practice in nursing care, maintaining and advancing own clinical competence.
•Participates in designated committees and research projects to advance the organization's goals and objectives.
