Job Summary:
We are seeking a highly skilled and compassionate Registered Practical Nurse to join our team as a Care Coordinator at Niagara Long Term Care Residence. As a key member of our healthcare team, you will play a vital role in coordinating and overseeing the delivery of care to our residents.
Key Responsibilities:
- Develop and modify care plans in response to residents' individualized health care needs and changes in health status
- Liaise with other healthcare personnel, agencies, and families to ensure seamless care delivery
- Maintain good relationships with medical, support, and home staff, residents, families, and the general public
- Supervise all activities on the unit, taking charge as required
- Monitor performance and complete performance evaluations for PSWs and RPNs
- Direct, assign, and evaluate the work of PSWs
- May be in charge of the Home as required, utilizing leadership skills to identify problems and problem-solving on the unit
- Monitor that care is delivered consistently with the resident Bill of Rights
- Required to understand the nature and meaning of quality indicators used by the Home and to understand which indicators are publicly reported
- Required to participate in the improvement of indicators and achieve satisfactory results
- Order and ensure nursing supplies and equipment are properly and efficiently used
- Actively listen to residents and family concerns, taking appropriate actions to resolve issues in accordance with policies of the Home
- Conduct unit rounds at the beginning and end of shift
- Process physician orders and administer medications and treatments in compliance with the policies of the Pharmacy provider, the Home, and the Standard of the College of Nurses
- Document and report all errors and omissions
- Maintain accurate and complete records of nursing observations and care in compliance with legislative requirements and the policies of the Home
- Assess changes in health status and report changes to Substitute Decision Makers (SDMs) or Power of Attorneys (POAs), and the interdisciplinary team
- Complete healthcare assessments (RAI-MDS and other assessments), develop and update plans of care for each resident in collaboration with the resident and/or SDM and the interdisciplinary team
- Demonstrate understanding of the Ministry of Long-term Care mandatory reporting and critical incidents and follow steps outlined in the policies of the Home
Qualifications:
- Current Certificate of Competence with the College of Nurses of Ontario as a Registered Nurse or a Registered Nurse registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60
- Basic CPR
- Experience in long-term care settings is an asset
- Ability to work with and relate to interdisciplinary team members
- Well-developed written and oral communication skills
- Knowledge of RAI-MDS – AIS certified, if not must be completed successfully within one (1) year of being hired
- Ability to show evidence of being focused on people's feelings when interacting and knowing how to turn a task into positive social interaction
- Ability to undertake and meet all legal requirements within a context of promoting individuals' rights and measuring/reducing elements of risk
- Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
- As a condition of employment, candidates are required to submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements
- Two supervisory references required
