Program Overview:

The LEAD (Let Everyone Advance with Dignity) initiative is a collaborative effort designed to enhance community resources aimed at improving public health and safety while reducing dependence on law enforcement and the judicial system for addressing public order challenges associated with substance use and sex work.

Role Summary:

The Community Outreach Coordinator will serve as the primary contact for all referrals directed to LEAD case management. This role is crucial for ensuring effective communication and collaboration among all stakeholders involved in the referral process. Key responsibilities include:

  • Acting as the main point of contact for all LEAD referrals.
  • Conducting initial assessments with new referrals to identify and address immediate client needs.
  • Engaging in street outreach to connect with individuals referred to the LEAD program.
  • Coordinating the assignment of new referrals to appropriate case managers.
  • Building and maintaining positive, collaborative relationships with LEAD partners and other service providers.
  • Documenting all activities in accordance with agency and contractual standards.

Qualifications:

A high school diploma or equivalent is required; an undergraduate degree is preferred. Relevant experience may be considered in lieu of a degree.

Skills and Knowledge:

  • Proven experience in providing outreach and engagement services in community settings.
  • Understanding of substance use disorders and harm reduction methodologies.
  • Strong interpersonal and communication abilities.
  • Excellent organizational skills with the capacity to prioritize tasks effectively.

Physical Requirements:

Employees may encounter exposure to illicit substances and must occasionally lift or move items weighing up to 30 pounds.
