About the Role

Villa Marconi Long Term Care Centre is seeking a highly skilled and compassionate Admissions Coordinator to join our team. As a key member of our admissions process, you will play a vital role in ensuring a smooth transition for new residents and their families.

Key Responsibilities
  • Resident Placement Coordination: Act as the primary point of contact for resident placement, working closely with Home and Community Care Support Services to ensure timely and efficient placement.
  • Admissions Process Management: Coordinate and facilitate the admissions process, including scheduling tours, conducting resident interviews, and completing necessary documentation.
  • Communication and Collaboration: Develop and maintain effective relationships with residents, families, and healthcare professionals to ensure a seamless transition.
  • Room Preparation and Maintenance: Ensure resident rooms are prepared and maintained to the highest standards, including coordinating with housekeeping and maintenance teams.
  • Exit Interviews and Feedback: Conduct exit interviews with discharged residents and families to gather feedback and identify areas for improvement.
  • Participation in Admission Day Conference: Participate in the admission day conference led by Social Work to ensure a comprehensive understanding of resident needs and preferences.
  • Business File Completion: Complete resident business files in a timely and accurate manner, ensuring all necessary documentation is in place.
  • Waiting List Management: Work with residents and families on the waiting list to ensure accurate and up-to-date information is gathered for business file completion.
  • Support and Advocacy: Provide support and advocacy for residents and their families, promoting a person-centered approach to care.
  • Communication with Public Guardian and Trustee: Communicate with the Public Guardian and Trustee regarding admission status and payment arrangements.
  • Collections Policy Enforcement: Ensure the collections policy is enforced when new admissions fail to make payment within 3 days of admission.
  • Quality Indicators and Performance Metrics: Understand and apply quality indicators and performance metrics to ensure high-quality care and services.
Requirements
  • Education and Certification: High School Diploma and Social Services Worker certification.
  • Experience and Skills: Previous experience in a geriatric social-related field, with excellent interpersonal and communication skills.
  • Electronic Health Records: Experience with electronic health records is preferred.
  • Language Skills: Second language is an asset.
  • Registration and Certification: Registered with the college annually and holds a current Vulnerable Sector Check (including a Criminal Background Check).
  • Health and Safety: Must provide a complete and current (within six months) TB testing or chest X-ray, as per Public Health requirements.
  • References and Vaccination: Three supervisory references required and COVID-19 vaccination is a condition of employment (2 doses mandatory).
