QUALIFICATIONS:

  • Must be a Registered Nurse in good standing with the College of Nurses of Ontario;
  • Baccalaureate education in nursing is preferred;
  • Membership in the Registered Nurses Association of Ontario is required;
* A minimum of 5 years of relevant clinical experience and proven clinical expertise in managing patient flow for individuals with complex discharge requirements.

  • Familiarity with community resources that facilitate discharges;
  • Ability to engage in critical discussions regarding transition planning with diverse patient populations;
* Comprehensive understanding of care levels and legislation affecting patients in hospital settings and the broader community;

  • Effective communication skills with patients, family members, interdisciplinary healthcare teams, and community partners;
  • Excellence in both oral and written communication;
  • Strong interpersonal, problem-solving, and teaching abilities;
  • Capacity to collaborate effectively within an interdisciplinary team;
  • Proven analytical and problem-solving skills in fast-paced environments such as emergency departments and inpatient units;
  • Extensive experience and knowledge in Gerontological Nursing and participation in quality improvement initiatives;
  • Proficient in computer applications, particularly EPIC, MS Office, and other relevant software;
  • Flexibility to support leadership, clinical teams, patients, family members, and community organizations across all Lakeridge Health sites;
  • Must maintain a satisfactory performance record;
  • Commitment to core values of Inclusion, Compassion, Innovation, Teamwork, and Joy;
  • Understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices, including participation in regular in-service education; and,
  • Proven good attendance record with the ability to maintain this standard.

CONDITIONS OF EMPLOYMENT:

Employment terms and conditions are governed by the Collective Agreement between the Hospital and the Ontario Nurses Association (RN Bargaining Unit Local 051).

Successful external applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.
