Palliative Care Navigator

We are seeking a skilled Palliative Care Navigator to join our team at Nova Scotia Health. As a Palliative Care Navigator, you will play a critical role in identifying clients' needs, developing recommendations, and evaluating the most appropriate services and programs to support individuals and families in living in their own community for as long as possible.
You will work collaboratively with clients' family physicians, multi-disciplinary healthcare teams, and community supports/agencies to ensure seamless access to services and programs that enable successful pain management for clients and their families.
The ultimate goal of this role is to empower clients to live with support and successfully meet their personal goals of care.
About YouWe are looking for a candidate with a degree in a healthcare discipline, such as nursing, occupational therapy, physiotherapy, or social work. You should be registered or eligible for registration with a governing body and have three years of experience working with Palliative care and community organizations and services.
You should have in-depth knowledge and experience with community practice and frail elderly populations, as well as excellent verbal and written communication skills. Ability to develop and maintain community networks, use data in the evaluation and improvement of the Geriatric Navigator, and be innovative and creative in developing strategies to meet the needs of diverse client, family, and provider populations.

HoursThis is a long-assignment, full-time position with 75 hours bi-weekly. The position is approximately 24 months in length, and dates are subject to change.
Compensation and IncentivesThe hourly rate for this position is $42.89. Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
