About the Opportunity

We are seeking a skilled Palliative Care Navigator to join our team. This is a full-time position, working 75 hours bi-weekly.

The successful candidate will work collaboratively with clients, family physicians, and healthcare teams to identify client needs, develop recommendations, and evaluate services that support individuals living in their own community for as long as possible.

This role requires a strong understanding of palliative care and community resources, as well as excellent communication skills. The ideal candidate will be able to develop and maintain community networks, use data to improve services, and be innovative in their approach.

Key Responsibilities
  • Assess client needs and develop recommendations for services and programs
  • Collaborate with healthcare teams and community organizations to provide support
  • Develop and maintain community networks to enhance services
  • Analyze data to inform service improvements
Required Skills and Qualifications
  • Degree in a healthcare discipline (e.g., nursing, occupational therapy, physiotherapy, social work)
  • Registered or eligible for registration with a governing body
  • Three years' experience working in palliative care and community organizations
  • In-depth knowledge of community practice and frail elderly population
  • Excellent verbal and written communication skills
  • Ability to develop and maintain community networks
Benefits

This position offers a competitive salary range of $33.97-$42.89 per hour, plus benefits including health, dental, travel, long-term disability, and life insurance coverage. Additionally, you may be eligible for a defined benefit pension plan.
