Job Summary

The University Health Network is seeking a highly skilled and experienced Pharmacist to join our Transplant Outpatient Pharmacy team. As a key member of our team, you will be responsible for facilitating program development and quality improvement activities, while exemplifying our values of caring, excellence, teamwork, innovation, integrity, leadership, and respect.

Key Responsibilities

  • Review and dispense medication orders for appropriateness
  • Identify, resolve, and prevent drug-related problems
  • Monitor drug therapy for effectiveness, toxicity, and adherence to evidence-based medicine and guidelines
  • Conduct medication reconciliation
  • Follow up on medication incidents and promote safe medication practices
  • Respond to drug information questions and provide direct and/or functional supervision of technician staff

Qualifications

  • Completion of a Bachelor's of Science Degree in Pharmacy or Doctor of Pharmacy degree
  • Certificate of competency from the Pharmacy Examination Board of Canada
  • Active registration with the Ontario College of Pharmacists (Part A)
  • Pharmacy Residency an asset
  • Ambulatory Outpatient Pharmacy practice experience an asset
  • Familiarity with common drug therapies and dispensing processes for specialty medication
  • Experience working in an ambulatory clinic or hospital environment, including developing a new clinical pharmacist role
  • Excellent verbal and written communication skills
  • Excellent customer service and interpersonal skills
  • Excellent organizational and time management skills
