Work Hours: 35 hours per week
Reports to: Director of Care Services
Position Summary: The Occupational Therapist within the Geriatric Assessment Intervention Network (GAIN) Community Team is tasked with conducting assessments using various standardized methodologies, applying clinical judgment, and creating tailored interventions for older adults and seniors experiencing the emotional, psychological, and physical effects of aging.
Key Responsibilities:

1. Engage as a geriatric assessor in inter-professional Comprehensive Geriatric Assessment (CGA) Conduct clinical triage for new referrals Gather and document data across all domains of Comprehensive Geriatric Assessment (CGA) 2. Deliver Clinical Care and Consultation Services to enhance and/or sustain daily living activities for frail older adults and seniors . Evaluate the capabilities and expectations of frail older adults and seniors concerning life activities through observation, interviews, and formal assessments Collaborate with the GAIN inter-professional team to develop care plans for patients Education Requirements: University Degree in Occupational Therapy Registered and in good standing with the College of Occupational Therapists of Ontario Membership with CAOT (Canadian Association of Occupational Therapists) or OSOT (Ontario Society of Occupational Therapists) is an asset Current CPR and First Aid Certification Experience & Skills: Two to three (2-3) years of relevant experience in the field Proven extensive knowledge and skills in working with individuals facing various types of dementia, delirium, mental health issues, addictions, responsive behaviors, other neurological conditions, and those undergoing acute behavioral changes Experience and knowledge in therapeutic engagement with geriatric populations is preferred Strong understanding of cognitive and mobility assessment tools Certification in Gentle Persuasive Approaches, PIECES, or Montessori is preferred Proficient in crisis intervention and assessment is preferred Initiative and adaptability are essential skills Excellent communication skills (both written and verbal) in English Proficiency in additional languages is desired (Armenian, Greek, Mandarin, Tamil, Hindi, Urdu, etc.) Additional Requirements: Vulnerable Sector Check is mandatory A valid driver's license and access to a vehicle for home visits are required Proof of full COVID-19 Vaccination or Medical Exemption Certificate is required Senior Persons Living Connected fosters a diverse work environment.
