Care Director

Key Responsibilities:- Assist the Director of Care in implementing and evaluating quality improvement initiatives, risk management systems, and data collection and analysis.
- Take the lead in coordinating and implementing best practice and risk management initiatives related to resident care, such as falls prevention, restraint use, wound management, and infection control.
- Effectively manage the utilization of nursing supplies and maintain a system of care assessment, planning, implementation, and evaluation at the resident and unit program level.
- Mentor staff and oversee the provision of quality resident care, while maintaining an active presence in the Resident Home Areas.
- Collaborate with the Director of Care on special projects, reviews, and other duties as assigned.
- Assist with input to Performance Appraisals and focus on Resident Centered Care Needs as required by MOH LTC Home Legislation and Extendicare Policies.
- Participate in the palliative care program, including goals of care and advanced care planning.
- Minimum of three years experience in long term care, with previous management experience an asset.
- Current knowledge of clinical issues for geriatric residents, infection control, principles of adult education, and quality improvement.
