Director of Care

At Granite Ridge Community, we are seeking a compassionate and dedicated Director of Care to lead our clinical team in providing high-quality care to our residents. As a key member of our leadership team, you will be responsible for ensuring excellence in resident care, meeting regulatory compliance, and leading, coaching, and engaging team members.
Key Responsibilities:- Ensure clinical excellence in resident care by working effectively with the nursing team and external partners.
- Monitor and ensure clinical care processes are delivered according to individual plans of care for residents.
- Lead, implement, and monitor key clinical programs, such as falls prevention, skin and wound care, continence care, and pain management.
- Coordinate, implement, evaluate, and monitor the performance metrics of care processes for RAI MDS Program, Nursing Rehab, and Restorative Care.
- Collaborate with Clinical Partners on issues related to infection control and coordinate monitoring of all activities at the Care Community.
- Completed a Bachelor of Nursing Science or equivalent post-diploma education.
- Currently registered and in good standing with the College of Nurses.
- Minimum 1 year experience working as a Registered Nurse in Long Term Care.
- Minimum 2-3 years experience working as a Registered Nurse in a managerial or supervisory capacity in a healthcare setting.
