At Extendicare, we are committed to delivering high-quality care to our residents. As a valued member of our leadership team, the RAI/MDS Coordinator will play a crucial role in ensuring the timely completion of MDS assessments and providing education to staff on data collection and reporting guidelines.

Key Responsibilities:
  • Coordinate the completion of MDS assessments on all residents within established timelines.
  • Provide leadership in streamlining MDS processes and integrating them with existing clinical tools.
  • Participate in strategic planning for program development and improvement, staying current on RAI trends.
  • Interpret reports generated by Health Zone/AH&W or CIHI and provide critical analysis for the home.
  • Coordinate the planning and organizing of the home RAI process integration and sustainability.
  • Provide on-site RAI MDS 2.0 education to all appropriate staff members as part of staff orientation and regular reviews.
  • Manage the coordination of and participating in the timely completion of Resident assessments, tracking forms, Resident Assessment Protocols (RAPs), and Resident Care Plans.
  • Assist RAI users in correctly coding assessments and provide ongoing support for validation of coding.
  • Oversee the RAI process according to home policies, provincial and regional regulations, professional colleges' Standards of Practice, and interRAI requirements.
  • Provide ongoing leadership and mentoring for education, data accuracy, data submission, and computer applications of the RAI process.
  • Implement a data quality program to ensure data accuracy and promote capacity building and staff competency.
  • In consultation with management and interdisciplinary team members, develop processes that ensure data integrity, efficient data collection, assessment completion, and data utilization.
Requirements:
  • 3-5 years' experience in Long Term Care, with experience with MDS an asset.
  • Current registration with the College of Nurses of Ontario and a member in good standing with applicable regulatory body.
  • Knowledge of InterRAI suite of tools, MDS/RUG III patient classification systems.
  • Demonstrated interest in the RAI Coordinator role.
  • Proven ability to get things done.
  • Demonstrated success leading new initiatives.
  • Well-developed oral and written communication skills.
  • Knowledge and experience with adult education strategies/facilitation.
  • Previous experience presenting to groups.
  • Well-organized, with excellent time management skills.
  • Clear aptitude for detail-oriented work.
  • The ability to work with and relate to interdisciplinary team members and other internal stakeholders.
  • Strong problem-solving abilities.
  • Proficient computer skills.
  • Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one.
