Position: Peer Support Specialist

STATUS: This is a temporary part-time role.

WORKING HOURS: 08:30 - 16:30
Work Days: Saturday to Tuesday
Days Off: Wednesday to Friday

COMPENSATION: $30.64 - $32.31 per hour

JOB OVERVIEW: The Peer Support Specialist (PSS) will report to the Clinical Operations Manager and will utilize their personal experiences related to living in challenging environments, mental health issues, and substance use to assist clients in overcoming barriers to healthcare. The PSS will work collaboratively with clients and the healthcare team to tailor care that meets individual needs.

RESPONSIBILITIES:

  • Establish supportive relationships with clients, ensuring proactive engagement.
  • Aid clients in accessing necessary health and social services as part of their care plan.
  • Accompany clients to appointments and assist in scheduling follow-ups.
  • Collaborate with the research team to enhance clinical follow-up.
  • Conduct outreach to clients for clinic appointments and home visits when necessary.
  • Coordinate with the interprofessional healthcare team to communicate outcomes and provide support.
  • Participate in community engagement events relevant to client goals.
  • Educate clients about their rights and grievance procedures within the healthcare system.
  • Provide individual and group support focused on empowerment and life skills.
  • Assist clients in community integration activities and accompany them to appointments as needed.
  • Advocate for clients and facilitate connections between community service providers and the healthcare team.
  • Share insights with the healthcare team regarding the experiences of individuals from marginalized communities.
  • Collaborate with clients and team members to identify goals and develop care plans.
  • Assist in de-escalating situations and engage outside resources when necessary.
  • Maintain accurate records using electronic charting systems.
  • Understand clinic protocols and relay this information to clients to improve access to care.
  • Engage in quality improvement initiatives related to client follow-up and retention.
  • Perform additional related duties as assigned.

QUALIFICATIONS:

  • Relevant experience in mental health, addiction, homelessness, or social services, including service planning and peer support training.
  • Current First Aid and CPR certification is an asset.

SKILLS AND KNOWLEDGE:

  • Understanding of the challenges faced by individuals with mental health issues, addiction, or homelessness.
  • Ability to act as a role model by sharing recovery experiences.
  • Flexibility and creativity in service delivery to diverse populations.
  • Skills in crisis resolution and effective communication.
  • Knowledge of harm reduction principles and community integration strategies.
