Job Summary

We are seeking a skilled Rehabilitation Specialist to join our team at BC Mental Health & Substance Use Services. As a key member of our inter-professional team, you will work collaboratively with patients, families, and healthcare professionals to provide comprehensive rehabilitation services in a minimum and medium security unit setting.

Key Responsibilities

  • Develop and implement individualized rehabilitation plans to address the assessed needs of patients, incorporating a wide range of group-based and vocational rehabilitation programs.
  • Work collaboratively with the inter-professional team to provide recommendations and participate in the development of individualized rehabilitative treatment plans for patients.
  • Conduct rehabilitation assessments and document patient progress, adjusting treatment plans as needed to ensure priority needs are met.
  • Prepare individual discharge reports to meet departmental documentation standards, summarizing information on patients' community living skills and independence levels.
  • Maintain outcome and workload measurement statistics, analyzing data to inform program development and evaluation.

Requirements

  • Graduation from a recognized recreational, rehabilitation, or related health or human services program, plus two years recent related experience or an equivalent combination of education, training, and experience.
  • Valid Food Safe Certificate.
  • Valid Class 4 BC Driver's License.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to deal with others effectively.
  • Awareness of and commitment to learning and understanding the Truth & Reconciliation Commission's 94 Calls to Action, In Plain Sight Report, BC's Declaration on the Rights of Indigenous Peoples Act, and other related reports.
  • Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada.
  • Knowledge of social, economic, political, and historical realities impacting Indigenous communities and familiarity with Indigenous Cultural Safety and anti-racism reports.
