Job Title: Unit Coordinator - Emergency Department

Job Summary:

The Unit Coordinator - Emergency Department is responsible for engaging in daily discharge rounds for admitted patients within the Emergency Department, maintaining thorough output, identifying and facilitating appropriate discharge planning, and identifying and facilitating the transfer of patients to other sites as required.

Key Responsibilities:

  • Engage in daily discharge rounds for admitted patients within the Emergency Department
  • Maintain thorough output and identify opportunities for improvement
  • Facilitate appropriate discharge planning and coordinate transfers to other sites as required
  • Work closely with the inter-disciplinary team, including nurses, therapists, and social workers, to ensure seamless patient care
  • Develop and maintain accurate and up-to-date patient records
  • Communicate effectively with patients, families, and healthcare teams to ensure timely and efficient care
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures

Requirements:

  • Registration with the College of Nurses of Ontario (in good standing)
  • Current BCLS and ACLS certification
  • Recent and relevant Charge Nurse experience (within the last 2-3 years)
  • Significant and recent experience in an Acute Medical Unit (within the last year)
  • Recent experience in Emergency/Critical Care/Intensive Care nursing (preferred)
  • Proven experience with staff scheduling, patient flow, and patient assignments
  • Bachelor of Science in Nursing (B.Sc.N) degree (preferred)
  • Demonstrated leadership, teaching, and mentorship skills
  • Effective organizational, planning, prioritization, and time management skills
  • Demonstrated ability to deal with multiple competing demands and maintain composure in stressful situations
  • Proven ability to work well independently and demonstrate well-developed critical thinking skills
  • Ability to collaborate with Lakeridge Health Patient Flow, inpatient clinical program interdisciplinary teams, and community partners for quality-based patient outcomes
