Community Access & Transition Nurse (RN/RPN)

As a dedicated Registered Nurse or Registered Psychiatric Nurse, you are looking for a fulfilling position that presents challenges and the opportunity to positively influence the lives of others. You will collaborate with a multidisciplinary team, employing trauma-informed approaches, harm reduction strategies, and ensuring Indigenous Cultural safety.
Key Responsibilities:
- Engage with healthcare team members to provide client transition support
- Verify demographic and health information of clients preparing for release
- Coordinate with community resources to develop effective transition plans
- Facilitate connections to community programs and services
- Contribute to team planning and quality enhancement initiatives
Qualifications:
- Graduation from an accredited School of Nursing with current registration
- A minimum of two years of relevant experience
- Completion of Occupational First Aid II certification
- Proficient in partnership development, application of nursing theories, and effective communication
- Dedication to Indigenous Cultural Safety and reconciliation efforts
